Service Forms

Please help us by using one of the following forms to submit information or service request. This lets us address matters in the order they were received and centralizes all activity in one location.
 

• Update My Rental Information
  Best way to notify us of changes to your information such as phone numbers, email address and billing changes.
   
• Report a Damaged Rental Bin
  If you are renting a cart from Herby Curby it is your responsibility to notify us if a cart is damaged.
   
• Report a Stolen Cart
  You need to file a police report before submitting this form.  A deductibe based on the age of the cart will have to be paid before a rental cart is replaced.  We do not replace stolen purchased carts.
   
• Request Repair Service
  Have a broken lid, wheel or other issues? Let us know so that we can repair it and prevent problems with pickup services.
   
• Cancel My Rental
  Even though we hate to see you go, we try to make the process as simple and easy as possible.
   
• Submit a Warranty Claim
  This form is for purchased bins only.  If you are renting from us please complete the form Report a Damaged Rental Bin. You will require the serial number of the cart before submitting the claim.
